STATE OF WEST VIRGINIA

        At a Regular Term of the Supreme Court of Appeals continued and held at Charleston, Kanawha County, on the 11th day of February, 2010, the following order was made and entered:


        RE: APPROVAL OF FORMS FOR DOMESTIC VIOLENCE CASES

        On a former day, pursuant to the Forms Update Protocol, came the Director of Court Services, Angela Saunders and Director of Family Court Services, Lisa Tackett, and presented to the Court proposed forms for use in domestic violence cases.

        Whereas upon consideration thereof, a completed Civil Case Information Statement Domestic Violence Cases as set forth in the prescribed form as an appendix hereto, will be required to accompany every Domestic Violence Petition filed; and

        Whereas, upon further consideration whereof, the Court is of opinion to and does hereby approve and adopt for mandatory use the Domestic Violence forms as set forth in the appendix to this order in their entirety. The Court directs the Administrative Office to distribute the forms and provide instructions for their use. The forms may be accessed and downloaded from the West Virginia Supreme Court of Appeals website at www.state.wv.us/wvsca.

        Now, therefore, it is hereby ORDERED that all domestic violence petitions be accompanied by a completed Civil Case Information Statement Domestic Violence Cases, in the form as set forth in the appendix hereto; and

        It is finally ordered that the Domestic Violence Forms set forth in the appendix to this order are approved and adopted for use in all Domestic Violence Cases.
