PEIA NOTICES


YOUR RESPONSIBILITY TO MAKE CHANGES

If you belong to the PEIA PPB Plan, a managed care plan, or have life insurance only coverage, it is your responsibility to update your PEIA enrollment records. For example, changes in your family, a new address, a change in marital status, coverage qualification for a dependent child, etc. 

Effective July 1, 2009, PEIA required documentation of every Change in Status. 
        Notify benefit coordinators Linda Foster or Amanda Fitzwater one to two months prior to any changes. Failure to promptly notify your benefit coordinator of changes in your family status may result in the Court requiring you to reimburse any premiums paid in error, and your plan may adjust claims paid for ineligible enrollees.

MOUNTAINEER FLEXIBLE BENEFITS (MFB)

To Make a Change:
        Within sixty days of an event that is consistent with one of the events listed above under PEIA, you MUST complete and submit a Change in Status/Election Form to Fringe Benefits Management Company (FBMC). Contact FBMC at 1-800-342-8017 to obtain this form.

MEDICAL IDENTIFICATION CARDS

PEIA PPB Plan:
        Effective July 1, 2009, Wells Fargo issued new ID cards to members for Plan Year 2010. Please destroy your old cards and begin using the new cards immediately. If you need a replacement card or additional cards, contact Wells Fargo at 1-888-440-7342.

Managed Care Plan:
        Managed care plans, Carelink and The Health Plan issue ID cards each year. You will receive an identification card from that plan, not from PEIA. For additional or replacement cards, contact Carelink at 1-800-348-2922 or the Health Plan at 1-800-624-6961.

DENTAL & VISION PLANS

There are NO ID cards distributed with these plans.

IMPORTANT REMINDER

Employee w/employee spouse (Two public employees who are married to each other, and who are eligible for benefits under PEIA)

Since July 1, 2006, the Family with Employee Spouse premium discount has not been granted unless both employees are basic life insurance policyholders in the plan.
