Many of the forms on this site are being made available as Adobe Acrobat ® Portable Document Format (PDF) files so that they can be printed and then used in the same manner as any traditional paper form.  They can be completed online and printed with the free Acrobat Reader®.  

  • You may complete the forms in any of three ways:

1Print and Complete: A form may be printed, then typed or filled in by hand.

2.  Complete On-Line: A form may be filled out while online, then printed.  Note that you risk losing your work of your browser crashes or your internet connection fails. 

3.  Save and Complete: If your browser is set to open Acrobat Reader within the browser window, you have access to the browser's "File - > Save As" feature.  Save the form to a folder on your computer; go off line; open Acrobat Reader as a separate program, the open and complete the form.  You will be able to print, but not save, the completed form. 

  • Tips for completing the form on your computer:

If you save the form to a folder on your computer, you can then go offline and open the form in Acrobat Reader.  The "hand" cursor will turn into a text tool when placed over a field in which you can type.  Click in the first text box with your mouse, type your information, then tab to the next field.  To return to a field later, click in the field or use shift-tab tab backwards.  Click in a box to put a check in it. 

To ease viewing while completing the form you can enlarge the form by clicking on it with Acrobat's magnifying glass. 

  • Forms cannot be sent electronically.  Print, sign and date the form, make necessary copies and place in the mail. 

Please note:  Some of the PDF forms on our site work best in Acrobat Reader 4.0 or higher.  If you experience difficulty completing a form on-line, please upgrade your version of Acrobat Reader.
